Applying is quick and easy. Complete each of the online forms below, and your responses will be submitted directly to Sirens & Sparkles First Aid CIC. Once we've reviewed your application, we'll be in touch to discuss the next steps.
A copy of each submitted form will be emailed to you for your records and automatically sent to Sirens & Sparkles First Aid CIC.

If you choose to pay by Direct Debit, the Direct Debit mandate included in this form will be completed and signed with you during your trial session before payments begin.

Payment Setup During the Trial
During your child or young person's second trial session, we'll discuss membership and payment options if a place is offered.
If you choose to pay by Direct Debit, the Direct Debit mandate included within the Youth Subscription Form will be completed and signed during your trial session before payments begin.
Membership payments begin from the third session if the place continues.
What Happens Next?
Once we receive all the required forms, we will review the application and contact you about the next steps. Please check that the email address entered on each form is correct so you receive a copy.

What Happens Next?
